1959 – May 31 stubs (2) – Koufax win.
1959 – June 22 stubs (3) – Koufax win (16 strikeouts).
1959 – June 27 stub – Koufax win.
1959 – September 29 FULL - Playoff pennant clincher vs. Milwaukee (Marked Game “A”).
1961 – Sept 15. stub – Koufax win vs Spahn (10 strikeouts).
1962 – July 4 stub – Koufax win (10 strikeouts).
1965 – World Series Game 5 stub - complete game shutout (10 strikeouts).
1966 – August 13 – FULL PSA VG 3 – Koufax 11 strikeouts, win #157/165, last season.
